You have to make arrangements ahead of time to get out to Cape Lookout, but it is worth the effort.  There are several charter services that will take you out, and at one time, there was a ferry under contract to the National Park Service.  The last time we were there the lighthouse was freshly renovated and repainted, and looked wonderful.  The keepers house was open, with docents staying there to guide us through.
